<strong>patologia infiammatoria<\/strong> multisistemica di natura cronica, la cui manifestazione patognomonica risiede nella formazione di <strong>granulomi epitelioidi <\/strong>non caseosi in molteplici organi e tessuti.<sup data-fn=\"9fe52fd5-884b-4eea-99fd-1310048b2fb4\" class=\"fn\"><a href=\"#9fe52fd5-884b-4eea-99fd-1310048b2fb4\" id=\"9fe52fd5-884b-4eea-99fd-1310048b2fb4-link\">1<\/a><\/sup> <sup data-fn=\"4a6ab215-7869-45ab-8860-e81980465bd0\" class=\"fn\"><a href=\"#4a6ab215-7869-45ab-8860-e81980465bd0\" id=\"4a6ab215-7869-45ab-8860-e81980465bd0-link\">2<\/a><\/sup><\/p>\n\n\n\n<p>Sebbene la presentazione clinica mostri una straordinaria eterogeneit\u00e0, il coinvolgimento dell&#8217;<strong>apparato respiratorio<\/strong>, in particolare dei <strong>polmoni <\/strong>e dei <strong>linfonodi intratoracici<\/strong>, si verifica in oltre il&nbsp;90%&nbsp;dei pazienti adulti, rendendo la gestione pneumologica il cardine del percorso assistenziale.<\/p>\n\n\n\n<p>La natura enigmatica della malattia risiede in un&#8217;eziologia ancora non del tutto chiarita, che sembra scaturire da una <strong>risposta immunitaria aberrante <\/strong>e persistente innescata da <strong>trigger ambientali<\/strong> o <strong>infettivi <\/strong>in soggetti caratterizzati da una specifica vulnerabilit\u00e0 genetica.<sup data-fn=\"3ab95872-579a-4d4d-b56d-6df1119622d5\" class=\"fn\"><a href=\"#3ab95872-579a-4d4d-b56d-6df1119622d5\" per la sua capacit\u00e0 di interessare quasi ogni organo, manifestandosi con sintomi che variano drasticamente in base alla sede e all&#8217;estensione del coinvolgimento granulomatoso.<\/p>\n\n\n\n<p>Circa un terzo dei pazienti presenta sintomi sistemici prodromici, tra cui <strong>astenia <\/strong>(che pu\u00f2 essere invalidante e non correlata all&#8217;attivit\u00e0 della malattia), <strong>febbricola<\/strong>, <strong>calo ponderale<\/strong> e <strong>sudorazioni notturne<\/strong>.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"apparato-respiratorio-90-303a0259-92c8-4f2d-b915-cf9156c8782a\"><span class=\"ez-toc-section\" id=\"Apparato_respiratorio_%3E90\"><\/span>Apparato respiratorio (&gt;90%)<span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n<div class=\"wp-block-image\">\n<figure class=\"alignright size-full\"><img loading=\"lazy\" decoding=\"async\" width=\"240\" height=\"240\" src=\"https:\/\/www.bmscience.net\/blog\/wp-content\/uploads\/2026\/01\/sarcoidosi.jpg\" alt=\"\" class=\"wp-image-26312\" srcset=\"https:\/\/bmscience.net\/blog\/wp-content\/uploads\/2026\/01\/sarcoidosi.jpg 240w, https:\/\/bmscience.net\/blog\/wp-content\/uploads\/2026\/01\/sarcoidosi-150x150.jpg 150w\" sizes=\"auto, (max-width: 240px) 100vw, 240px\" \/><\/figure>\n<\/div>\n\n\n<p>La sarcoidosi polmonare si presenta tipicamente con <strong>tosse <\/strong>secca e stizzosa, <strong>dispnea da sforzo <\/strong>che pu\u00f2 progredire verso la <strong>dispnea a riposo<\/strong> nelle forme avanzate, e <strong>dolore toracico aspecifico<\/strong>.<\/p>\n\n\n\n<p>All&#8217;esame obiettivo, il torace pu\u00f2 risultare sorprendentemente silente, anche in presenza di estese alterazioni radiografiche, sebbene in alcuni casi siano udibili fini <strong>crepitii basali<\/strong>.<\/p>\n\n\n\n<p>L&#8217;<strong>emottisi<\/strong> \u00e8 estremamente rara e deve far sospettare complicanze come il micetoma in cavit\u00e0 fibrotiche preesistenti.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"manifestazioni-cutanee-25-4b143337-49f6-4801-8760-fbd500201855\"><span class=\"ez-toc-section\" id=\"Manifestazioni_cutanee_25\"><\/span>Manifestazioni cutanee (25%)<span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p>La pelle rappresenta una &#8220;finestra&#8221; diagnostica fondamentale. Le lesioni si dividono in specifiche (contenenti granulomi) e aspecifiche:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>eritema nodoso:<\/strong>&nbsp;manifestazione aspecifica caratterizzata da noduli rossi, caldi e dolenti sulle superfici estensorie delle gambe. \u00c8 spesso associato a una prognosi favorevole e fa parte della sindrome di L\u00f6fgren;<\/li>\n\n\n\n<li><strong>Lupus Pernio:<\/strong>&nbsp;placche rosso-violacee indurite localizzate sul naso, guance e orecchie. \u00c8 un segno di malattia cronica, spesso associato a coinvolgimento delle vie respiratorie superiori e a una risposta terapeutica meno brillante;<\/li>\n\n\n\n<li><strong>lesioni cicatriziali:<\/strong>&nbsp;i granulomi possono infiltrarsi in vecchie cicatrici o tatuaggi, causandone il rigonfiamento La recidiva \u00e8 possibile in circa il&nbsp;5%&nbsp;dei soggetti che hanno precedentemente raggiunto la remissione.<\/p>\n\n\n\n<p>L&#8217;identificazione di<strong> fenotipi ad alto rischio<\/strong> \u00e8 essenziale per un intervento precoce. La gestione richiede programmi di riabilitazione polmonare e, in casi selezionati, l&#8217;uso di neurostimolanti come il <strong>metilfenidato <\/strong>o l&#8217;<strong>armodafinil<\/strong>.<\/p>\n\n\n\n<p>Un&#8217;altra complicanza emergente \u00e8 la sindrome delle apnee ostruttive del sonno (<em><strong>OSAS<\/strong><\/em>), che mostra una prevalenza aumentata nei pazienti sarcoidei e contribuisce significativamente alla sensazione di stanchezza diurna.<\/p>\n\n\n\n<p>Il monitoraggio a lungo termine deve essere individualizzato, con visite ogni&nbsp;3\u22126&nbsp;mesi per i pazienti attivi e controlli annuali per quelli in remissione stabile.Gli esami di routine includono prove di funzionalit\u00e0 respiratoria, calcemia\/calciuria, ECG e valutazione oculistica.
