{"id":5485,"date":"2015-01-07T19:26:47","date_gmt":"2015-01-07T18:26:47","guid":{"rendered":"http:\/\/www.bmscience.net\/blog\/?p=5485"},"modified":"2024-03-10T12:18:09","modified_gmt":"2024-03-10T11:18:09","slug":"lesame-delle-urine","status":"publish","type":"post","link":"https:\/\/bmscience.net\/blog\/lesame-delle-urine\/","title":{"rendered":"L&#8217;esame delle urine"},"content":{"rendered":"\n<p>L\u2019esame delle&nbsp;<strong>urine<\/strong>&nbsp;\u00e8 un test di laboratorio di notevole importanza; il campo di indagine che viene offerto da questa tipologia di esame \u00e8 estremamente vasto; molte delle indagini che vengono effettuate sulle urine ci danno tutte le indicazioni utili non solo nella diagnosi di diversi&nbsp;<a href=\"http:\/\/www.bmscience.net\/blog\/apparato-urinario-malattie-problemi\/\">disturbi renali<\/a>, ma anche informazioni riguardanti le patologie.<br>Com\u2019\u00e8 noto, l\u2019urina \u00e8 un liquido prodotto dai&nbsp;<strong><a href=\"http:\/\/www.bmscience.net\/blog\/struttura-interna-ed-esterna-rene\/\">reni<\/a><\/strong>, i quali hanno il compito di filtrare il sangue eliminando le sostanze di scarto. Tale liquido si accumula nella vescica e viene eliminato all\u2019esterno attraverso l&#8217;uretra tramite la minzione. Generalmente, nell\u2019arco delle ventiquattro ore, vengono eliminati circa 1,5 l di urine.<br>L\u2019esame standard delle urine comprende un&nbsp;<strong>esame chimico-fisico<\/strong>&nbsp;e un&nbsp;<strong>esame microscopico<\/strong>.<\/p>\n\n\n\n<p><strong>ESAME FISICO:<\/strong><\/p>\n\n\n<div class=\"wp-block-image\">\n<figure class=\"alignright size-large is-resized\"><img loading=\"lazy\" decoding=\"async\" width=\"1024\" height=\"1024\" src=\"https:\/\/www.bmscience.net\/blog\/wp-content\/uploads\/2024\/03\/Ketonuria1-1024x1024.jpg\" alt=\"\" class=\"wp-image-20039\" style=\"width:307px;height:auto\" srcset=\"https:\/\/bmscience.net\/blog\/wp-content\/uploads\/2024\/03\/Ketonuria1-1024x1024.jpg 1024w, https:\/\/bmscience.net\/blog\/wp-content\/uploads\/2024\/03\/Ketonuria1-300x300.jpg 300w, https:\/\/bmscience.net\/blog\/wp-content\/uploads\/2024\/03\/Ketonuria1-150x150.jpg 150w, https:\/\/bmscience.net\/blog\/wp-content\/uploads\/2024\/03\/Ketonuria1-768x768.jpg 768w, https:\/\/bmscience.net\/blog\/wp-content\/uploads\/2024\/03\/Ketonuria1-1536x1536.jpg 1536w, https:\/\/bmscience.net\/blog\/wp-content\/uploads\/2024\/03\/Ketonuria1.jpg 1971w\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" \/><\/figure>\n<\/div>\n\n\n<p>L\u2019esame fisico \u00e8 la prima parte di una analisi delle urine ed \u00e8 un osservazione visiva diretta di 5 parametri:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>aspetto (colore, torbidit\u00e0, presenza di sedimento);<\/li>\n\n\n\n<li>densit\u00e0 (peso specifico);<\/li>\n\n\n\n<li>odore;<\/li>\n\n\n\n<li>osmolarit\u00e0;<\/li>\n\n\n\n<li>volume urinario delle 24 ore (diuresi).<\/li>\n<\/ul>\n\n\n\n<p class=\"has-text-align-left\">Normalmente le urine fresche sono limpide e presentano un colore che va dal<span style=\"color: #ffcc00;\"> giallo paglierino <\/span>al <span style=\"color: #ff9900;\">giallo arancio<\/span>. Le urine di colore <span style=\"color: #ff0000;\">rosso<\/span> o <span style=\"color: #993300;\">rosso-marrone<\/span> potrebbero essere causate da un colorante alimentare ingerendo bietole fresche, da un farmaco ( clorochina, ibuprofene, L-dopa ec&#8230;) o dalla presenza di emoglobina o mioglobina. Se il campione contiene molti globuli rossi, le urine sono torbide e rosse. Infatti la presenza eccessiva di materiale cellulare o proteine influenza la torbidit\u00e0 delle urine oltre che a comportare lo sviluppo di cristallizzazioni o sedimenti.<\/p>\n\n\n\n<p class=\"has-text-align-left\">La <strong>densit\u00e0<\/strong> (o peso specifico) delle urine \u00e8 determinato dalla quantit\u00e0 di sostanze sciolte nelle urine. In condizioni fisiologiche normali va da 1007 a 1030 (quando \u00e8 compreso tra questi valori si dice <strong>normostenuria<\/strong>). La densit\u00e0 diminuisce nei casi di <em>insufficienza renale cronica<\/em>, <em>diabete insipido<\/em> ecc. mentre pu\u00f2 aumentare in caso di patologie come il <em>diabete mellito scompensato<\/em>, la <em>glomerulonefrite<\/em> ecc.<\/p>\n\n\n\n<p class=\"has-text-align-left\"><span id=\"result_box\" class=\"\" lang=\"it\"><span class=\"hps\">Per quanto riguarda la diuresi, generalmente il volume di urine delle 24 ore va dai<\/span> <span class=\"hps\">750 ai 2000<\/span> <span class=\"hps\">ml<\/span> <span class=\"hps\">\/<\/span> <span class=\"hps\">24 ore<\/span><\/span><span class=\"\" lang=\"it\">. Un valore di urina al disotto di questi valori \u00e8 detta oliguria, mentre si parla di poliuria quando l&#8217;urina \u00e8 eccessiva.<\/span><\/p>\n\n\n\n<p class=\"has-text-align-left\">Il <strong>pH<\/strong> delle urine pu\u00f2 risultare pi\u00f9 alcalino (basico) in condizioni di digiuno e pi\u00f9 acido dopo i pasti. Il range di normalit\u00e0 va da 4,6 a 7,8. Valori inferiori o superiori rispetto al range di normalit\u00e0 possono essere spia di alterazioni dell\u2019equilibrio acido-base. L\u2019<strong>acidit\u00e0 urinaria<\/strong> pu\u00f2 essere dovuta a <em>calcoli<\/em>, <em>dieta iperproteica<\/em>, assunzione di farmaci a base di <em>metionina<\/em> o <em>cloruro di ammonio<\/em> ecc.\u00a0Le urine possono risultare alcaline quando si segue una dieta eccessivamente ricca di <em>agrumi<\/em>, nel caso di <em>alcalosi respiratoria<\/em>,<em> alcalosi metabolica<\/em> ecc.<\/p>\n\n\n<div id=\"bmscience3996628212\" style=\"margin-top: 15px;margin-bottom: 15px;margin-left: auto;margin-right: auto;text-align: center;\"><div data-id='24157' class='amazon-auto-links aal-js-loading'><p class='now-loading-placeholder'>Now loading&#8230;<\/p><\/div>\r\n\r\n<\/div>\n\n\n<p class=\"has-text-align-left\"><strong><span id=\"result_box\" lang=\"it\" class=\"\" style=\"font-size: 14pt;\">ESAME CHIMICO:<\/span><\/strong><\/p>\n\n\n<div class=\"wp-block-image\">\n<figure class=\"alignright size-large is-resized\"><img decoding=\"async\" src=\"https:\/\/learn.parallax.com\/sites\/default\/files\/content\/Reference\/Urinalysis\/Multistix-Urinalysis-Color-Key.png\" alt=\"\" style=\"width:288px;height:auto\"\/><\/figure>\n<\/div>\n\n\n<p class=\"has-text-align-left\"><strong><\/strong>Attraverso l\u2019esame chimico delle urine si ricerca la presenza di glucosio, proteine, chetoni, sangue urobilinogeno, bilirubina ecc. Esso viene effettuato per mezzo di una striscia multistick.<\/p>\n\n\n\n<p class=\"has-text-align-left\">L\u2019esame delle urine \u00e8 un esame poco costoso, indolore e non invasivo (tranne che nei casi di ritenzione urinaria, dove si deve ricorrere all\u2019introduzione di un catetere nell\u2019uretra per poter prelevare il liquido urinario) che pu\u00f2 portare al riconoscimento di varie patologie sia relative agli apparati renale e urinario sia ad altri organi o apparati.<\/p>\n\n\n\n<p class=\"has-text-align-left\">Come detto, il campo di indagine delle urine \u00e8 eccezionalmente vasto ed \u00e8 praticamente impossibile in questa sede dettagliare accuratamente tutti i parametri e gli analiti che possono essere presi in considerazione durante l\u2019esame delle urine. Di seguito illustreremo quindi i pi\u00f9 comuni.<\/p>\n\n\n\n<p class=\"has-text-align-left\"><strong>Bilirubina<\/strong> \u2013 Normalmente la bilirubina dovrebbe essere assente nelle urine (o, al limite, presente in quantit\u00e0 piccolissime). Cause della presenza di bilirubina nelle urine potrebbero essere patologie quali la cirrosi epatica, neoplasie del pancreas, anemia perniciosa, anemia emolitica, talassemia, sepsi ecc.<\/p>\n\n\n\n<p class=\"has-text-align-left\"><strong>Chetoni<\/strong> \u2013 I corpi chetonici (acido acetico, acido \u03b2-idrossi-butirrico e acetone) sono prodotti intermedi del metabolismo lipidico. Di norma sono assenti nelle urine. La ricerca di corpi chetonici \u00e8 una prova di laboratorio utile nella diagnosi del diabete mellito. Si possono avere falsi positivi dopo il test alla bromosulfonftaleina.<\/p>\n\n\n<div id=\"bmscience3134751670\" style=\"margin-top: 15px;margin-left: 15px;float: right;\"><a href=\"https:\/\/amzn.to\/40yDUd0\" target=\"_blank\" aria-label=\"B1UNwQRyIDL (1)\"><img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/bmscience.net\/blog\/wp-content\/uploads\/2025\/07\/B1UNwQRyIDL-1.gif\" alt=\"\"  width=\"300\" height=\"169\"   \/><\/a><\/div>\n\n\n<p class=\"has-text-align-left\"><strong>Emoglobina<\/strong> \u2013 L\u2019emoglobina \u00e8 di norma assente nelle urine. Cause della sua presenza possono essere l\u2019infarto renale, farmaci, ustioni, setticemia ecc.<\/p>\n\n\n\n<p class=\"has-text-align-left\"><strong>Glucosio<\/strong> \u2013 Il glucosio \u00e8 un monosaccaride che, in condizioni fisiologiche normali, non dovrebbe essere presente nelle urine. Se vi \u00e8 presenza di glucosio nelle urine e i valori della glicemia superano il range di normalit\u00e0 potremmo trovarci di fronte a patologie quali il diabete mellito, la sindrome di Cushing, tumore al pancreas, feocromocitoma, ipertiroidismo ecc.<\/p>\n\n\n\n<p class=\"has-text-align-left\"><strong>Nitriti<\/strong> \u2013 La rilevazione di nitriti nelle urine \u00e8 indice di infezioni a livello delle vie urinarie.<\/p>\n\n\n\n<p class=\"has-text-align-left\"><strong>Proteine <\/strong>\u2013 Normalmente le proteine dovrebbero essere assenti nelle urine. Talvolta per\u00f2 se ne rileva una minima quantit\u00e0 per motivi non patologici (per esempio uno sforzo fisico non minimale). Una presenza di proteine superiore ai 150 mg\/l potrebbe essere dovuta a glomerulonefrite, cistite, diabete mellito, gravidanza, ipertensione, mieloma ecc.<\/p>\n\n\n\n<p class=\"has-text-align-left\"><strong>Pigmenti biliari <\/strong>(urobilina e urobilinogeno) \u2013 La presenza di pigmenti biliari nelle urine oltre i valori normali (0,5-2,5 mg nella raccolta di urine delle 24 ore) potrebbe indicare epatopatie di varia natura o un\u2019ostruzione delle vie biliari.<\/p>\n\n\n<div id=\"bmscience1817783583\" style=\"margin-top: 15px;margin-bottom:
